Lead Instructor Registration

This form should be completed by the lead instructor or club secretary on behalf of the lead instructor.

Your invoice number may also be referred to as your ‘order number’ or ‘reference number’. It will be a four or five digit number, usually prefixed with a hashtag – such as #3811. You don’t need to include the hashtag – just the number.

This membership option includes a contribution from us to the Fighting Chance Foundation to change lives through martial arts here in the UK. It also includes a donation to WaterAid to help provide people around the world with clean drinking water. Furthermore, this membership is Carbon Neutral – we’ll plant trees to offset the carbon footprint of every membership.
